What was the actual cause of the crash?

Exactly! I saw the report and it was a total system failure. The autopilot software got confused by a faded lane marker, and the driver wasn't paying attention at all. It's that simple combo that did it. Both parts of the system, the car and the person, messed up at the same time. Makes you wonder how many people are just blindly trusting that tech. Really basic stuff that shouldn't have happened.
