Update: Tracing a faulty zone back to a crimped cable was my lightbulb moment.
I mean, idk, maybe it's just me but now I run a continuity test on every run before termination.

Five years ago, I would terminate cables without a second thought. It seemed like extra work for no real gain. Then I spent a whole weekend chasing a ghost fault in a system, only to find a crimp like yours. That was the day I stopped cutting corners. Now the tester comes out before any connectors go on. It saves more time than it costs, which is the kind of math I can get behind.

How many hours have we all wasted on stuff like that? That weekend of your life is gone forever, and it was over a bad crimp. The tester is cheap compared to your time. I don't even think of it as extra work anymore, it's just part of the job. Skipping it feels like asking for trouble.
